Important update: As of October 2024, permit applicants are encouraged to use the division’s new electronic submission process to submit Modeling Requirements Determination Form APCD-114.
As a courtesy, the division will continue to accept the Modeling Requirements Determination Form APCD-114 (August 2024) as a PDF through Jan. 6, 2025. After that date, permittees must submit Form APCD-114 using the new electronic submission process.

The CDPHE Air Pollution Control Division forms and documentation located on this page are not specific to certain permits. This page is relevant for anyone looking to make an administrative change to an air permit that is required for compliance.

- Note: As of August 2024, use the online submission tool to submit a Notice of Startup form.
- Go online to submit the Notice of Startup form within 15 days of starting the operation/equipment or permit issuance.
- Create an account using the online submission tool or login to an existing account using the online portal.
- After Dec. 31, 2024, all Notice of Startup forms must be submitted using the online submission tool.
Alternate Operating Scenario (AOS) documentation

- Submit the cancellation request form electronically to cancel a construction permit or Air Pollutant Emission Notice (APEN) when:
- Equipment producing air emissions has been removed, replaced or sold.
- The operations that created emissions have ceased.
- The operations or equipment producing air emissions no longer require a permit or APEN, due to regulatory changes.
- Download or print the APCD-107: Cancellation request.
- To cancel a Title V Operating Permit or withdraw an application follow the cancellation request instructions.
Application rejection
- The division will reject an APEN if it is filled out incorrectly, missing information, or lacks payment for the filing fee. The re-submittal will require a new filing fee. Visit the division’s web page for application rejection frequently asked questions for more information.
- Submit the transfer of ownership form to transfer ownership of emission point(s) to a new owner. An Air Pollutant Emission Notice (APEN) must also be submitted for each individual active and reportable emissions point to be transferred.

Relocation notice
- To report relocation of sources that have been permitted as portable sources: 1) create an account using the online submission tool and 2) submit the relocation notice using the online submission tool.
- After Sept. 30, 2024, all relocation notices must be submitted using the new online submission tool.
- The division must receive this notice at least 10 days before the relocation.
- Only sources permitted as "portable sources" should use this form.
