Air ambulance services are licensed and regulated by the Colorado Department of Public Health and Environment. A license is required when patients are transported from any point within Colorado, to any point within or outside of Colorado. For emergency transport information call: 303-503-1263.
Air ambulance application steps
Establish a profile gatekeeper

Designate a person to be the Organizational Profile Account Gatekeeper who will submit and regularly update your organizational profile.

Have the designated Organizational Profile Account Gatekeeper apply for the role by following the organizational profile account gatekeeper instructions.

Once the gatekeeper role has been established in OATH they must submit the Organizational Profile Information form.
Submit an air ambulance application

The profile gatekeeper must contact the department to have the air ambulance application pushed to their OATH account. Call 303-503-1263 to have the application pushed to the gatekeepers OATH account.

Fill out and submit the Air Ambulance License application in OATH.

Pay the proper air ambulance licensing fees to the department.

Once the complete application is processed, an email will be sent to the gatekeeper confirming the application has been approved.
Choosing an air ambulance licensing path
There are 3 different methods of gaining an air ambulance license in Colorado.
- Accreditation by one of three approved organizations
- Colorado state inspection licensing
- Out-of-state recognition license
