Air ambulance licensing pathways and fees

Accreditation by approved organizations

This licensing pathway requires that you are fully accredited by a national accreditation organization. If your air ambulance agency is fully accredited by one of the following, you will be required to upload your accreditation certificate in your OATH air ambulance license application. See the fee chart below to determine the cost of this pathway.

Colorado state licensing inspection

This licensing pathway requires that your air ambulance agency receive an on-site inspection by a Colorado Department of Public Health and Environment inspector. The inspection process will be scheduled after you submit your OATH application for an air ambulance license and payment of fees. See the fee chart below to determine the cost of this pathway.

Out-of-state recognition license

This licensing pathway requires that you don't have a base in Colorado but have an air ambulance license from another state. Colorado will recognize the license and allow 12 flights originating in Colorado a year. Air ambulance agencies that may exceed 12 flights in a year must apply using either method above. See the fee chart below to determine the cost of this pathway.

Air ambulance licensing fees

The department has set the following fees for each licensing pathway. Each licensing pathway has a specific duration. Fees must be mailed to the department after submitting your application.
