Situations that require a substantial change to the project statement of work require prior approval from the EMTS funding program manager.
These situations may include:
- A functional equipment change. Example: you would like to change the type of equipment that was requested and approved in your statement of work.
- A request to reallocate items budgeted in one grant line item to another. Example: you've found that your costs for supplies are less than the quoted amount and would like to reallocate those savings to a line item that costs unexpectedly more.
- A change in specified equipment type. Example: you would like to change the equipment type listed in the statement of work to accomplish the project goal.
- A decrease in the amount awarded. Example: you've experienced savings in your project and will revert over $5000. An increase in your grant award cannot be requested.
Submit the Statement of Work Change Request form on this page to request a change to the activities found in the project's statement of work. Staff will review your request to determine if the application justifies the need for the required change.
