Ground ambulance licensing in Colorado
County governments currently conduct licensing of ground ambulance services. Effective July 1, 2024, the department will gain the authority to license ground ambulance agencies statewide. All ground ambulance services in Colorado must submit an onboarding application in the OATH Portal before May 1, 2024. Use the "Check license status" link to find the onboarding status for active Colorado ground ambulance services. This list is updated every Monday.
To find specific county ambulance licensing requirements and verification of a valid license before July 1, 2024, contact the pertinent county commissioner's office. Ground and air ambulance services are required to collect and submit patient care data to the department through the Colorado ImageTrend Elite web portal.
Ground ambulance licensing onboarding
All Colorado county-licensed ambulance services must submit a "Ground Ambulance License - Onboarding" application to the department by May 1, 2024. The Organizational Profile Account Gatekeeper for each county-licensed ambulance service must log into OATH to submit the onboarding application. Use the "Check license status" link to find the onboarding status for active Colorado ground ambulance services. This list is updated every Monday.
Before starting the onboarding application, ensure that your ambulance service has:
- A designated organizational profile account gatekeeper
- A current organizational profile in OATH
- A PDF copy of your current county licenses displaying an expiration date of June 30, 2024 or later.
- A medical director who is registered as your agency medical director in OATH
Steps to submit the onboarding application
- The profile gatekeeper must log into their OATH account.
- Click Applications on the left menu.
- Click the View Services Applications button next to the ambulance service name.
- Click the Apply Now button next to the "Ground Ambulance License - Onboarding" application.
- Be sure to check every ambulance VIN to ensure accuracy

- Complete the application form and submit it for review.
