Ground ambulance policy templates
The policy templates below are available for your agency’s use and may be downloaded and edited as needed. Templates dated July 1, 2024, reflect the current requirements. Templates dated July 1, 2026, pertain to policies under the delayed implementation timeline and are effective beginning July 2026. Each template includes a reference to the applicable rule at the top of the document in a box with blue text. This reference is included for your convenience and may be retained or removed as needed. To assist with clarity and customization, the templates use a color-coded system:
- Blue text indicates the required language
- Black text offers suggested language
- Red text designates areas where agency-specific information must be inserted
