Critical Events System

Colorado’s Critical Events Program is an innovative approach to addressing barriers to support a person living with HIV to engage in care. This program requires case managers to work in collaboration with the client to develop a plan of action to specifically address barriers to accessing and engaging in care. Leveraging State and Federal funds, our program assists clients in attaining stability and achieving viral suppression.   

Requirements

  • The client must be nominated for the program and meet eligibility criteria to be accepted into the program. 
  • This assistance is not just an urgent fund for clients. It must go toward a plan that will help them move toward stability within a six-month timeframe.
  • This program requires a high degree of collaboration with funded contractors and key stakeholders to support the client to achieve their goals.
  • Keeping track of the client's progress is vital to this program.

    • Module 1: Introduction to the CDPHE Office of STI/HIV/Viral Hepatitis Critical Events Program
      • Program The Introduction to the CDPHE Office of STI/HIV/Viral Hepatitis Critical Events (CE) Program module provides an overview of the CE Program. The overview will cover a brief history of the program, describe what services can and cannot be provided through the program, and outline program eligibility criteria and expectations. Beyond the minimal program eligibility and expectations, this course will explore for whom CE would be an appropriate resource. Using a job aid flowchart, a client will be followed from eligibility assessment through nominations, acceptance, time in the CE Program, and program discharge.
