A person living with HIV or AIDS is healthier and is less likely to transmit their infection when their viral load is suppressed or undetectable. With consistent care and prescribed drugs, clients can reach viral suppression. It's hard for many of these people to keep their healthcare because of changes in their personal lives. Our program uses funding from the Affordable Care Act and Medicaid expansion to help clients move toward stability.
The client must be nominated for the program.
This assistance is not just an urgent fund for clients. It must go toward a plan that will help them move toward stability within a six-month timeframe.
Every agency and staff person involved in a client's treatment plan must share this devotion to the client's stability.
Keeping track of the client's progress is vital to this system.
