Colorado’s Critical Events Program is an innovative approach to addressing barriers to support a person living with HIV to engage in care. This program requires case managers to work in collaboration with the client to develop a plan of action to specifically address barriers to accessing and engaging in care. Leveraging State and Federal funds, our program assists clients in attaining stability and achieving viral suppression.
Requirements
- The client must be nominated for the program and meet eligibility criteria to be accepted into the program.
- This assistance is not just an urgent fund for clients. It must go toward a plan that will help them move toward stability within a six-month timeframe.
- This program requires a high degree of collaboration with funded contractors and key stakeholders to support the client to achieve their goals.
- Keeping track of the client's progress is vital to this program.
