- Become a customer of the State Lab if you have not submitted tests previously.
- Check that your test can be ordered in LabOnline. If not, request an updated paper requisition form.
- In the form, choose “Other” in the update/change field and request a manual requisition form and the email to send it to you in the Change description field.
- Next, sign up for our electronic data submission portal called LabOnline.
- LabOnline is a secure, self-service online lab portal where you can order lab tests and view results processed by the State Lab.
- Order testing supplies by placing a request using the appropriate form.
- Review the specimen collection and storage methods. Follow labeling and packaging instructions to prevent delays in test processing and receiving results.
- Request a courier to transport your sample(s) to the State Lab.
