2026-2027 Annual Parent Letter for child care and preschool:  

The Annual Parent Letter explains immunization requirements for school entry. 

By law, child cares are required to directly distribute the Annual Parent Letter to the parent(s) or legal guardian(s) of every enrolled student by April 15, 2026.

Child cares are required to include their school’s MMR and exemption rates from the 2024-2025 school year. Child cares required to report data to CDPHE may find 2024-2025 school year data at COVaxRates.org.

Notice of Exclusion

School and child care professionals may use this form to notify families that their student will be excluded from school or child care until updated documentation has been received at the school or child care.
