Colorado

Get a copy of your immunization records from the Colorado Immunization Information System (CIIS). 

A U.S. state or territory other than Colorado 

Contact the Immunization Information System (IIS) in the city, state, or territory where the student received their last vaccines. Some have a public immunization record request portal similar to Colorado.

You can also work with your Colorado health care provider, as CIIS has interjurisdictional data exchange with some states, including Kansas, Nevada, New Mexico, Oklahoma, and Pennsylvania.

A country outside of the U.S., or if you cannot find the student’s immunization record.

Contact the student’s immunization provider or their previous school.

What if you don’t find your records?

If records are lost, children or adults may need to repeat some vaccinations. It is generally not harmful to receive additional doses. Your health care provider can help you make a plan.
