2026-2027 Annual Parent Letter for school (K-12):

The Annual Parent Letter explains immunization requirements for school entry. 

By law, schools are required to directly distribute the Annual Parent Letter to the parent(s) or legal guardian(s) of every enrolled student by April 15, 2026.

Schools are required to include their school’s MMR and exemption rates from the 2024-2025 school year. Schools required to report data to CDPHE may find 2024-2025 school year data at COVaxRates.org

Updated Tdap letter for the 2026-2027 school year

Starting in the 2026-2027 school year, Colorado law will require students to receive a dose of Tdap vaccine before their first day of seventh grade.

This is a change from the 2025-2026 school year and before, when Colorado law had required one dose of Tdap vaccine before sixth-grade entry.

Notice of Exclusion

School and child care professionals may use this form to notify families that their student will be excluded from school or child care until updated documentation has been received at the school or child care.
