International travel for health care providers
Are you interested in becoming a yellow fever vaccine provider in the state of Colorado?
Review the Colorado State Yellow Fever Vaccination Center Certification Policy to determine if you meet the eligibility requirements and begin the certification process.
How to become a Colorado Yellow Fever Provider:
- Submit a Yellow Fever Vaccination Center Application online for each center enrolling.
- A maximum of five yellow fever vaccination centers may be registered under one medical provider.
- Complete the CDC’s Yellow Fever Vaccine: Information for Health Care Professionals Advising Travelers training course online.
- Any staff advising travelers on the vaccine on behalf of the provider should also complete the course.
- Upload a signed Yellow Fever Certification Provider Agreement.
- Upload a signed Yellow Fever Storage and Handling Agreement.
- Upload a data logger report with five days of temperatures for the unit that will hold the yellow fever vaccine.
- Upload the data logger calibration certificate.
Once the online application and documents have been received, the provider will be sent an email confirming their certification and given instructions on how to purchase an official stamp and order the vaccine from Sanofi.
If you are an existing yellow fever provider and need to recertify, follow the steps outlined above for enrollment. Recertification is due every three years. For more information about recertification, review the Colorado State Yellow Fever Vaccination Center Certification Policy.
