Funding opportunity details

Request for Applications #11460 is a combined effort from multiple chronic disease prevention and management programs at the Colorado Department of Public Health and Environment to promote and improve chronic disease and health outcomes among underserved Coloradans. This funding opportunity incorporates many of the services provided through the WISEWOMAN program, the Clinic Quality Improvement for Population Health Initiative and the Diabetes and Cardiovascular Disease Prevention and Management program.=

Funding is not currently available for the Community Outreach Strategy of Track 1: WISEWOMAN Clinical and Outreach Services. Applications for this strategy will still be accepted and held until additional funding becomes available. Application responses for the Community Outreach Strategy will not be reviewed or scored until funding becomes available. Applications WILL be reviewed and scored for other Track 1 strategies. Track 1: WISEWOMAN Clinical and Outreach Services is now open to Applicants that are not currently funded through the Cancer Prevention and Early Detection (CPED) Program to conduct breast and cervical cancer screening activities under the Women’s Wellness Connection (WWC) program. 
Track 2: Clinical-community Linkages Health System Healthy Behavior Support Services Referral Strategy is now open to Applicants proposing to: 

  • Establish bi-directional referral relationships with any health system. Priority given to Applicants seeking to work with current Chronic Disease Prevention and Management Track 1 or Track 3 health systems. 
  • Build capacity to implement an approved healthy behavior support service. 
  • Train providers of Chronic Disease Prevention and Management Track 1 or Track 3 health systems to implement an approved healthy behavior support service.
