Request for Applications #11460 is a combined effort from multiple chronic disease prevention and management programs at the Colorado Department of Public Health and Environment to promote and improve chronic disease and health outcomes among underserved Coloradans. This funding opportunity incorporates many of the services provided through the WISEWOMAN program, the Clinic Quality Improvement for Population Health Initiative and the Diabetes and Cardiovascular Disease Prevention and Management program.=
Funding is not currently available for the Community Outreach Strategy of Track 1: WISEWOMAN Clinical and Outreach Services. Applications for this strategy will still be accepted and held in case additional funding becomes available. Application responses for the Community Outreach Strategy will not be reviewed or scored unless funding becomes available. Applications will be reviewed and scored for other Track 1 strategies.
