The public health testing supply order form is available for use by local public health agencies, epidemiologists, and health care professionals only (used for all public health testing and for all patient testing).
Information for health care professionals
- Rectal, pharyngeal, and vaginal specimens are now being accepted for Chlamydia trachomatis and Neisseria gonorrhea testing
- Collection of rectal and pharyngeal specimens for CT/GC APTIMA testing
- Tuberculosis sputum collection kit and shipping instructions
