NCAA/Sickle Cell Requests/Parent Sample Requests

To protect your privacy, request NCAA/Sickle Cell newborn screening results directly from the birthing facility or your child’s primary care physician. They will coordinate with the Colorado newborn screening team and have the results faxed or emailed to them. If the newborn screen was completed outside of Colorado, contact the state where the test was conducted. Providers requesting results on behalf of patients who no longer reside in Colorado, a medical release form signed by the patient or the parent/legal guardian of the child is required in addition to a completed Results Request Form. 

We ask that all requests allow up to five business days for processing.
