How to apply for a Colorado medical marijuana card

Apply for or renew your medical marijuana registry card

Step 1: See your health care provider 

You need a new provider certification each time you apply. Your health care provider will write it for you and submit it electronically. Finish the rest of your application within 6 months of the date you get your provider certification.

At your appointment

  • Get a printed copy of the provider certification for your records.
  • Ask your health care provider to check that all information on your certification is correct. 
  • Submit a new provider certification each time you apply.

Applicants age 18-20

  • All minor patients must see two providers and submit two physician certifications.
  • If the minor patient has Post Traumatic Stress Disorder, Autism Spectrum Disorders, or any condition for which a physician could prescribe an opioid, and the recommending provider is not the patient’s primary care physician, the physician should review the records of a diagnosing physician or a licensed mental health provider acting within his or her scope of practice. Refer to C.R.S. 25-1.5-106 and House Bill 19-1028.

Applicants aged 18-20

  • Applicants in this age group that did not have a registry card before age 18 and applied for the first time on or after Jan. 1, 2022 will need provider certifications from two different providers at different medical practices each year when they apply.
  • If the applicant is homebound, the providers do not need to be from different medical practices. Make sure both providers enter their certifications into the Medical Marijuana Registry system.

Step 2: Complete your your application online 

Make sure you have all the required information. 

Enter your information

  • Renewal applicants log into your account and click on your name under "Tracking Inbox: Patient Registration." Then scroll down and click "Save and Start Application." 
  • First-time applicants create an account and click “New” under "Tracking Inbox: Patient Registration." 
    When you have entered and checked your information, click "Save & Submit Patient Application."Applications must be submitted by the patient or the patient's legal representative.

Add your provider certification

  1. Click “Provider certification” 
  2. Click “New”
  3. Choose your provider certification from the drop-down menu
  4. Click “Save”

Applicants age 18-20

Applicants in this age group that did not have a registry card before age 18 and applied for the first time on or after Jan. 1, 2022 are required to submit two provider certifications. Currently, the system will only allow you to add one certification to your application. We will add the other one for you after you submit your application. Please make sure all of your identifying information matches on both provider certifications.

Choose your cultivation information

  1. Click  the “Cultivation information” link
  2. Click “New”
  3. Select your cultivation option and complete the required fields
  4. Click “Save”

If you are applying with a caregiver, make sure that you have your caregiver's registration ID number. Your caregiver will need to accept a caregiver agreement to be listed on your card.

Pay 

The application processing is fee is $29.50. It is non-refundable and is required each time you submit an application.

By credit card, debit card, or electronic check 

  • Click "Payment"
  • Click "New"
  • Choose "Click here to pay." This will open a new window where you will enter your payment information. 

Fee waiver

You may qualify for a fee waiver if your household income is 185% of the Federal Poverty Level or less.

To apply with a fee waiver, submit a copy of your certified Colorado tax return for the current tax year. You can get this from the Colorado Department of Revenue.

Step 3: We review your application 

Applications submitted online with all correct information are approved in 1-3 business days.It will take longer to process your application if it is submitted with incomplete pr expired information. 

You will get in email after we process your application. Check the email you used to register.

Step 4: Print or Display your card

Once approved, you can access your card through your online account.

  • Click on your name under "Tracking Inbox: Patient Registration."
  • Click the “print card” button to get a PDF of your card that you can print or display on a mobile device.
  • If you applied online you will not receive a card in the mail. 

The issue and expiration dates on your card will be based on the day your application was approved.

If you need another copy of your provider certification:

  1. Log in to your patient account by entering your username and password.
  2. Under "Tracking Inbox: Patient Registration", click on your name next to “Active”
  3. Select the "Provider Certification" tab on the top row of tabs
  4. Click on your most recently issued provider certification
  5. You can then select “Print Friendly Format” to view a PDF of the certification and print it if you need to.

The Marijuana Enforcement Division (MED) at the Colorado Department of Revenue rules clarify what information is required when designating a primary medical marijuana center. Please contact the Marijuana Enforcement Division if you have questions about this requirement.

  • A valid Colorado ID or driver's license. If you are applying online, make sure to have a JPEG or PDF copy of your physical, valid Colorado driver’s license or ID. to upload to your application. We do not accept Colorado digital IDs from the MyColorado app.
  • Your social security number.
  • Caregiver’s registration ID if you are applying with a caregiver.
  • Credit card or bank account and routing number to pay the $29.50 application processing fee.

Applicants 18-20 years old

  • Minor's social security number.
  • Primary parent's social security number.
  • Certified copy of the minor’s state-issued birth certificate. If you need more information about certified documents please review our tips for submitting certified documentation.
  • Caregiver’s registration ID if parent is not the caregiver.
  • Minor patient consent form | Formulario de consentimiento para paciente menor de edad
  • Documentation to prove secondary parent status:
    • Secondary parent’s out of state ID if parent lives out of state.
    • Copy of secondary parent’s certified death certificate if parent is deceased.
    • Copy of a certified court-issued sole custody order if the secondary parent does not have any custody. If you need more information about certified documents please review our tips for submitting certified documentation
  • Credit card or bank account and routing number to pay the $29.50 application processing fee.

  • Caregiver’s registration ID if the legal representative is not the caregiver.

Marijuana Enforcement Division's Uniform Certification Form

if a patient's provider authorizes them to purchase more than the statutory daily sales limit, they will need a Uniform Certification Form from the Marijuana Enforcement Division that has been completed by their recommending provider. The patient does not need to include this form with their application for a medical marijuana registry identification card as it is separate from the provider certification that is submitted to the Registry.

Qualifying medical conditions

Debilitating medical conditions

  • HIV or AIDS
  • Cachexia
  • Persistent muscle spasms
  • Seizures
  • Severe nausea
  • Severe pain

Disabling medical conditions

  • Post Traumatic Stress Disorder (PTSD)
  • An Autism Spectrum Disorder
  • Any condition for which a physician could prescribe an opioid

Residency requirements

Adults

  • To be eligible for a medical marijuana card you must:
  • Be a Colorado resident
  • Be 18 or older
  • Have a qualifying medical condition

Minors

Minors are eligible for a medical marijuana card if:

  • Minor and the primary parent are Colorado residents
  • Minor has a qualifying medical condition​
  • All minor patients must see two providers and submit two provider certifications.

Mail applications

Paper mail-in applications are reserved for applicants who do not have access to any internet resources.

Apply online

Complete and send the following documents through certified mail

 

Important information

  • It will take approximately 6-8 weeks to process your mailed application.
  • All correspondence with the registry will take place by mail. This means it may take 6-8 weeks to receive a response each time you contact the registry.
  • Your physician must submit your certification online and provide you with a printed copy for your records. 
  • If you have a caregiver they must be registered in the online system and provide you with their caregiver ID to enter on your application.
  • Applicants who apply by mail will be mailed a card, and will not have access to their online account.
