Viral Respiratory Vaccination Status Interview

The Colorado Department of Public Health and Environment (CDPHE) is one of 12 sites in the Emerging Infections Program with the Centers for Disease Control and Prevention (CDC). The program collects information on certain infections to learn more about disease patterns and risk factors. We use data from this program to make health recommendations and better understand infectious diseases in Colorado and the rest of the country.

CDPHE may contact individuals recently hospitalized with influenza or respiratory syncytial virus (RSV) to collect important information on their vaccination status. The information provided will help CDPHE understand who is getting ill and why. More importantly, it can help prevent other people from becoming ill. FAQs

How did you get my information?

Colorado law requires health care providers, hospitals, and laboratories to report certain conditions to CDPHE so we can follow up with patients to help monitor and inform disease activity in our state. This information is confidential and stored in a secure manner (just like the information you share with your doctor).

Why should I complete the survey?

By providing information on influenza or RSV immunization products received or not received prior to a hospitalization, you’re helping public health gain more information about disease risk.

What kinds of questions will you ask?

The interview asks about influenza or RSV immunization products received prior to hospitalization and, if received, the date.

What will you do with my information?

All responses are confidential and stored in a secure manner (just like the information you share with your doctor). Your information will only be used by public health to help determine disease risk in the community.

How long will it take? 

The survey generally takes only a couple of minutes. Your assistance is greatly appreciated and critical to understand illness and help prevent others in your community from getting sick.

How do I complete the interview? 

If you or your child were recently hospitalized for influenza or RSV, you may be contacted via text or email with a secure link to a website called REDCap to complete an online survey.
