About

This initiative engages with health systems to assess current performance on select health measures, increase capacity to monitor performance, identify public health focus areas, and implement targeted evidence-based interventions proven to improve health outcomes for selected focus areas.

Initiative areas of focus and associated measures:

  • Colorectal cancer screening (NQF 0034).

  • Breast cancer screening (NQF 0031/2372).

  • Cervical cancer screening (NQF 0032).

CDPHE partners with the University of Colorado Practice Innovation Program to provide practice transformation support and quality improvement expertise, and to deliver the CQI programs for colorectal, breast, and cervical cancer screening.

For general program information, please contact Zerimar Soto at zerimar.soto@state.co.us. For information about participating in the CQI program, please contact support@practiceinnovationco.org.

Colorado Community Health Network (CCHN) and Colorado Community Managed Care Network (CCMCN) will provide practice transformation and data collection support to participating clinics.
