Colorado Medical Home Community Forum
- The Colorado Medical Home Community Forum provides an opportunity to meet and learn about statewide medical home programs and initiatives in Colorado.
What is Medical Home?
Medical Home is one of eight statewide priorities for the Maternal and Child Health block grant. In addition to the state-level work, five local public health agencies (LPHAs) are also working on the medical home priority focused on aligning their HCP care coordination services with the Regional Accountable Entity (RAE) care coordination services for children enrolled in Medicaid.
