Health Care Coalitions
Health Care Coalitions (HCCs) are a network of health care and responder organizations within the HCC-defined boundaries that work together to prepare for threats to the health care system. These organizations coordinate resources to meet needs related to patient transport, placement, or care. Membership includes hospitals, Emergency Medical Services (EMS), emergency management, public health, behavioral health, and ancillary health care providers. HCCs are solely funded through federal Hospital Preparedness Program (HPP) grant dollars, which support the coordination and operational response activities. While HCCs prepare for any threat, the most common coordination activities are in response to fires, floods, power outages, extreme weather, or supply chain issues.
There are nine Health Care Coalitions in Colorado. Each coalition manages its own membership and activities.
North Central Region
Adams, Arapahoe, Boulder, Broomfield, Clear Creek, Denver, Douglas, Elbert, Gilpin, and Jefferson counties
Northeast Region
Sedgwick, Phillips, Logan, Morgan, Weld, Larimer, Washington, Yuma, Kit Carson, Cheyenne, and Lincoln counties
Northwest Region
Eagle, Garfield, Grand, Jackson, Mesa, Moffat, Pitkin, Rio Blanco, Routt, and Summit counties
San Luis Valley Region
Saguache, Mineral, Rio Grande, Alamosa, Conejos, and Costilla counties
South Region
Fremont, Custer, Pueblo, Huerfano, and Las Animas counties
South Central Region
Lake, Chaffee, Park, Teller, and El Paso counties
Southeast Region
Crowley, Kiowa, Otero, Bent, Prowers, and Baca counties
Southwest Region
Montezuma, La Plata, Archuleta, San Juan, and Dolores counties
