You have the right to get the information you need to make informed healthcare decisions. Colorado law (Section 25-58-101, C.R.S.) requires health facilities to provide you with information on the availability of services and procedures related to
- Reproductive healthcare services
- LGBTQIA+ healthcare services
- End-of-life healthcare services
The availability of these services is shown in a service availability form.
The service availability form will be published for facilities to download and complete June 17th, 2024. These forms must be sent to the Department at cdphe_saf@state.co.us on or before August 16, 2024. The Department will display completed forms on or before October 1, 2024.
