Health Facilities Data Request Form

Please continue to the attached Google Form to request data from our Health Facilities data team. Be prepared to provide the purpose of this request, the variables of interest, and a brief summary of the request.

Please note, due to recent staffing changes and increased demand on the data team, please anticipate a delayed response in fulfilling your data request. This will be added to the queue and the data team will complete the request in the order that it was received.
