The Special Manual Record Correction Request section in Chapter 5 was updated with v1.19.1 of the MDS 3.0 RAI Guidance Manual effective October 1, 2024, adding new rules for how to remove an MDS record from iQIES when the submitted record was “not for OBRA or Medicare Part A purposes.” The new rules address erroneous standalone assessments, as well as erroneous assessments that are combined with OBRA-required assessments.
A Manual Deletion Request is Required in Only Four Circumstances:
- Item A0410 Submission Requirement is incorrect
- Record was submitted for the wrong facility
- Record submitted was not for OBRA or Medicare Part A purposes
- Inappropriate submission of a test record as a production record
Once the Manual Correct/Delete form has been completed, it is sent via certified mail through the USPS to:

Additionally, a notification with the facility name, facility ID, contact person (email and telephone number), the date the letter was mailed, and certified tracking number will be sent to the State RAI Coordinator by selecting the link and completing the attached form.
After receipt of the request the information will be reviewed, the form will be signed, and forwarded to iQIES via certified mail through the USPS. Once iQIES processes the request, a notification will be sent to the provider via email.
DO NOT SEND/MAIL THE FORM DIRECTLY TO IQIES. IT MUST BE SENT THE STATE AGENCY FOR VERIFICATION AND SIGNATURE.
