The Gamete Bank and Fertility Clinic Program licenses and regulates gamete banks, gamete agencies, and fertility clinics that operate in Colorado or that operate outside of Colorado and provide gametes or embryos from unknown donors to recipient parents located in Colorado.

This table lists gamete banks, gamete agencies, and fertility clinics that meet compliance standards set by 5 CCR 1005-6 and have been licensed by the Colorado Department of Public Health and Environment.

Both of the following tables list all of the current licenses that have been issued by the Department. The tables will be updated as licenses are added or removed.

The following table lists entities that have received a provisional license. These provisional licenses were issued because the required date to have a license is July 1, 2025, and the entity’s application is still being processed by the Program.
