The Gamete Bank and Fertility Clinic Program investigates concerns of noncompliance of 5 CCR 1005-6.

The Program only investigates complaints that are related to the collection, matching, and distribution of gametes (sperm and oocytes) from unknown donations, to recipients in Colorado. If your complaint is not regarding a gamete bank, gamete agency, or fertility clinic, refer to the Health Facilities complaints page.

Filing anonymously

We accept anonymous complaints. If you file a complaint, your information is not shared with the entity or anyone else. In some cases, your identity may be discovered due to the nature of the complaint. When filing anonymously, please provide enough information to enable us to effectively investigate the complaint, as we will not be able to contact you for further information.
