Grant Project Transition Announcement

On August 21, 2023, the Tobacco Education, Prevention, and Cessation Grants Program at the Colorado Department of Public Health and Environment’s Prevention Services Division released a Request for Applications for Transition of a Project to fund a pilot a healthy housing program in Southwest Colorado. to address healthy housing and commercial tobacco-related inequities. The purpose of this STEPP application opportunity is to transition a previously-funded pilot project that will explore community-level collaborations and policies to reduce exposure to health hazards in the home environment and improve living conditions that can reduce health inequities.

Approximately $100,000 annually is available through this transition opportunity. The funding cycle for this project is for state fiscal years 2024-2026. San Juan Basin Public Health was previously awarded this grant funding, beginning on July 1, 2023. Due to the dissolution of the public health department, San Juan Basin Public Health will be unable to support this work after December 31, 2023. STEPP is looking for an entity to continue the project for the remainder of the funding period (January 1, 2024 - June 30, 2026). STEPP reserves the option to shorten or extend awards.  

Due to the intended catchment area for the project as awarded, the only eligible entities to submit for transition of the project are: San Juan County, La Plata County, and Archuleta County. STEPP encourages collaboration across multiple entities; however, only one entity will be awarded the transition of the project.

The deadline to apply is September 11, 2023 at 5 p.m. 

Who should apply

This Call for Transition is limited to public or governmental entities that are based in Archuleta, La Plata, and San Juan counties.

Questions and Answers

STEPP will host a webinar on Thursday, August 24, 2023 at 12 p.m. to provide an overview of this request for transition of project and to answer questions from potential applicants.
