Reminders
  • Please let us know of any changes by submitting a letter of intent. Existing facilities can use the health facilities portal. We will send you the application for the change and help you through the process.
  • Applicants have 14 days to respond to issues during the licensing process. Unresponsiveness is cause for application denial.
  • Initial and change of name applications will be reviewed to ensure the facility's name does not create confusion or misrepresent the services that are offered.
  • Certificates of compliance are processed by the Department of Public Safety.
