- Please let us know of any changes by submitting a letter of intent. Existing facilities can use the health facilities portal. We will send you the application for the change and help you through the process.

Applicants have 14 days to respond to issues during the licensing process. Unresponsiveness is cause for application denial.

Initial and change of name applications will be reviewed to ensure the facility's name does not create confusion or misrepresent the services that are offered.
