PSD Contracts and Procurement supports other Prevention Services branches and their partners by managing the contracts and procurement procedures necessary to fund the goods and services needed to implement division strategies.

Entities wishing to do business with the State of Colorado may need to submit one or more of these documents:

• W-9 form. (PDF)
• Sample ACORD Certificate of Liability Insurance form. (PDF)
• Sample signature authority letter. (Word doc)
• Statement of Work (SOW) and Budget Development Checklist. (PDF)
• Lawful Presence Affidavit. (PDF)
