Fill out the Records Request form

Send your request in writing to cdphe_cora_hmwmd@state.co.us. We’ll respond to all requests within three business days. For the fastest response and most complete search results, include as much information as you have available, including:

  • Name of site/facility.
  • Street address/location. If a street address is not available, provide the nearest intersection or latitude/longitude coordinates.
    • HMWMD does not record parcel numbers and cannot search those values.
  • If requesting a specific record, include the title and date of document.  
  • Any additional information about the site or incident that may assist staff in their research.

Each environmental division maintains separate records.  You must contact the Air Pollution Control Division and the Water Quality Control Division individually to review their records.

Providing records

  • The Records Unit will provide a link to a search result or a single document.
  • Hard copy records are marked as the Document Type “Discrete Docs” in search results.
    • CDPHE charges the actual per-box retrieval fees for retrieving responsive records that are stored offsite.
  • The Records Unit will scan hard copy documents upon request. The first hour of work is performed free of charge; for jobs that require additional time, a fee of $41.37 per hour is charged per the Department Open Records Policy.
  • You may review records on site by appointment.
  • We often file multiple related documents in PDF portfolios. If you see the below image, simply download the document to view it.
