The CCPD Grants Program funds programs across the state that work to reduce chronic disease in Colorado by focusing on prevention, early detection and treatment for cancer, cardiovascular disease and pulmonary disease. CCPD grantees work cohesively across evidence-based strategies included within the FY19-21 CCPD Strategic Framework, within Policy and Environmental, Community-clinical Linkages, Health Systems Transformation and Workforce Development domains.
FY19-21 funding cycle
CCPD grants are competitive grant awards, and are funded on a three-year cycle. The current funding cycle is for state fiscal years 2019-2021(July 1, 2018 - June 30, 2021). Currently, the CCPD program funds 22 community grantee projects, as well as evaluation, surveillance and grantee training and technical assistance efforts.
