FY24-26 funding cycle
CCPD grants are competitive grant awards, and are typically funded on a three-year cycle. The upcoming funding cycle is for state fiscal years 2024-2028 (July 1, 2023 - June 30, 2028). The CCPD Grant Program seeks to build a cohesive, comprehensive approach to reducing chronic diseases in Colorado by focusing on prevention, early detection, and treatment for cancer, cardiovascular disease, and pulmonary disease. The grant program funds strategies across the CCPD Review Committee Priorities and Strategic Framework, within Policy, Systems & Environmental Change, Prevention, and Cross-Cutting Clinical Systems Quality Improvement paths while also allowing for evidence-informed innovation (Attachment I) to advance the work at a population level. Guidance documents for each path are as follows: Path 1: PSE, Path 2: Prevention, and Path 3: Cross-Cutting.

FY19-21 funding cycle & FY22-23 extension funding
The CCPD Grant Program RFA #6519 funds projects across the state that worked to reduce chronic disease in Colorado by focusing on prevention, early detection, and treatment for cancer, cardiovascular disease, and pulmonary disease. CCPD grantees worked cohesively across evidence-based strategies included within the FY19-21 CCPD Strategic Framework, within Policy and Environmental, Community-Clinical Linkages, Health Systems Transformation, and Workforce Development domains.
CCPD RFA #6519 grants were competitive grant awards and were funded on a three-year cycle for state fiscal years 2019-2021 (July 1, 2018 - June 30, 2021). The CCPD RFA #6519 grant program funded 22 community grantee projects, as well as evaluation, surveillance, and grantee training and technical assistance efforts.
