Fiscal Agent for Health Care Coalitions (HCC)

A Request for Applications (RFA) is used to award grants to promote specific program goals. An organization (in this case, the Colorado Department of Public Health and Environment) announces that grant funding is available and eligible organizations may apply for it. The purpose of this CDPHE RFA is to identify Fiscal Agents for the nine Regional Health Care Coalitions (HCC). The federal Hospital Preparedness Program grant supports HCC efforts to help prepare for, respond to, and recover from emergencies that impact the medical system including EMS, hospitals, other licensed healthcare facilities, RETACs, LPHAs and Emergency Management. In Colorado, CDPHE serves the Public Health and Medical ESF 8 function and HCCs support that medical component at the regional level.  

 

Eligibility:

Organizations that may apply for funding must meet the following criteria:

  1. Fiscal Agent must agree to perform all components of the Scope of Work(s) in partnership with the HCC members.
  2. Fiscal Agent must be located within Colorado.
  3. Fiscal Agent must be either affiliated with or be a member of the healthcare industry. A member of the healthcare industry is defined as an entity which provides service to the medical system.
  4. Fiscal Agent must have the ability to, upon award, become an active member of the HCC. To be an active member, the awarded Fiscal Agent must be able to attend at least 75% of regularly scheduled HCC general meetings.
  5. Upon award, Fiscal Agent must have a demonstrated fiscal agent relationship with the desired HCC(s).

More details about your RFA:The Fiscal Agent is responsible for:

  • Using grant funds to support the equivalent of at least (1) FTE to fulfill Readiness Response Coordinator and Clinical Advisor responsibilities. 
  • Creating a sub-awardee agreement to allow funding the HPP grant activities in the Statement of Work and WorkPlan. The Work Plan will be created in collaboration with the HCC representatives and CDPHE. 
  • Managing the day-to-day fiscal activities of the HCC in compliance with required activities in the WorkPlan.
  • Provide to the HCC Steering Committee a monthly budget update.
  • Facilitating budget workshops to plan for new budget periods.
  • Reimbursing HCC member organizations for pre-approved purchases in a timely fashion.
  • Submitting standardized invoices to CDPHE for reimbursement.

 

Note: This RFA does not support funding research, clinical care, furniture or equipment, construction, lobbying activities, or vehicle purchase.


All aspects of the project are dependent upon funding availability. One application may be completed for multiple HCCs. Each HCC will need a separate budget.
