Planning phase: reports, forms, and templates
The following documents are for Planning-to-Operations grantees to complete in order to fulfill contractual obligations during the planning phase of the contact period.
In order to receive funding for the operations phase, the following forms and documents must be submitted to SBHC Program staff by the contract deliverables due date.
Transition to the Operations phase
As soon as a new SBHC is operational, planning-to-operations grantees will need to refer to the requirements for grantees of operating/existing SBHCs.
