Planning phase: reports, forms, and templates
The following documents are for Planning-to-Operations grantees to complete in order to fulfill contractual obligations during the planning phase of the contact period.
- Staff update form.
- Lessons learned template.
- Community Needs Assessment template(found on CASBHC site under Module 2/Community Assessment).
- General Survey Tools(found on CASBHC site under Module 2/Community Assessment).
- SBHC Operations Plan SAMPLE template (Note: Do not complete this template. This is just a SAMPLE and the Operations Plan should be completed in the Apex Hub).
In order to receive funding for the operations phase, the following forms and documents must be submitted to SBHC Program staff by the contract deliverables due date.
- Attestation for Memorandum of Understanding (MOU) form.
- Sample organizational structure diagram.
- SBHC Program Quality Standards Checklist.
- Complete the SBHC Operations Plan in the Apex HUB by the contract deliverable due date. Contact SBHC Program staff to add your Planning-to-Operating site to the Apex HUB in order to complete the Operations Plan.
Transition to the Operations phase
As soon as a new SBHC is operational, planning-to-operations grantees will need to refer to the requirements for grantees of operating/existing SBHCs.
