Health Facility Service Availability

This web page displays the service availability forms from the following licensed health facility types located in the State of Colorado:

  • General and Critical Access Hospitals
  • Community Clinics
  • Rehabilitation Hospitals
  • Freestanding Emergency Departments
  • Licensed Hospital Units

You have the right to get the information you need to make informed healthcare decisions. Colorado law (Section 25-58-101, C.R.S.) requires health facilities to provide you with information on the availability of services and procedures related to 

  • Reproductive healthcare services 
  • LGBTQIA+ healthcare services 
  • End-of-life healthcare services

The availability of these services and referrals for these services are shown in a service availability form. For the purposes of the service availability form, a “referral” is when a health-care professional or facility suggests that you see a different health-care professional or facility which offers the specific medical service you are looking for. Sometimes, this can be a written order from a doctor. Other times, this can be a list of health-care providers you get from a health facility.

The service availability form was published for facilities to download and complete June 17th, 2024. New and updated forms must be sent to the Department at cdphe_saf@state.co.us. As forms are processed by the Department, they will be posted to this webpage. All received forms will be posted on or before October 1, 2024.
