The Change of Ownership Guidance and Instructions document will provide information about the following topics:

The Letter of Intent (LOI) process requires two separate steps for a CHOW.

Step 1:
The current owner/seller is required to submit the LOI Form to notify the Department of a proposed CHOW. This will prompt the Department to send instructions to the current owner/seller on the process for granting the proposed owner/buyer access in COHFI.

Step 2:
After the current owner/seller has granted the proposed owner/buyer access in COHFI, the proposed owner/buyer will be able to complete the remaining steps of the CHOW process, including submitting the CHOW request, application, and fees. The department will work with the proposed owner/buyer to complete the application process.

  • Medicaid Certification - Home and Community Based (HCBS) Waiver Services/Specialties & Application Processes
  • The CDPHE Health Facilities and Emergency Medical Services Division does not charge fees for Medicaid Certification applications, however Health Care Policy and Financing may charge a fee for their Provider Enrollment process.
  • Not all HCBS programs require an application to be submitted to CDPHE. Please review the requirements specific to the service you are wanting to apply for.
