Please only submit the Letter of Intent (LOI) form if you have reviewed all requirements for licensure and you are prepared to begin the application process. 

If you are only looking for information on the process, timeframes, or requirements please review the information in the following sections prior to submitting the LOI.

If you do not know what provider type you should be applying for, you should not submit the LOI.
