What is a C&T?

A Certification & Transmittal, or a C&T, is a document created by the Health Facilities and Emergency Medical Services Division (HFEMSD) of the Colorado Department of Public Health and Environment (CDPHE) in order to communicate recommendations for certification actions to Health Care Policy and Financing (HCPF). The C&T document is provided to either the Facility/Agency, or in some cases directly to HCPF. Typically, the facility/agency that is applying for Medicaid approval must attach the C&T document to their facility/agency’s Medicaid Provider Enrollment Application during the HCPF enrollment process.

Where can I find Health Care Policy and Financing (HCPF) Requirements for Home and Community Based Medicaid Waiver services?
Here is the Department of Health Care Policy and Finance's list of all services/specialties with initial enrollment and revalidation requirements.

All applicants must complete the Medicaid Provider Enrollment Application in addition to the CDPHE HFEMSD requirements listed. HCPF requires a separate application for each physical location where Medicaid Waivers services are provided. 

All Home and Community Based Services are listed below with their HCPF specialty code, specialty name, and waivers where that service is included.

Specialty name: Adult Day Services

Waivers: (BI/CIH/CMHS/EBD)


Q: Is a CDPHE HFEMSD license required?
      A: No

Q: Do I need to submit an application to CDPHE?
      A: Yes, a Letter of Intent (LOI) for Adult Day Services and an application for Adult Day Services is required. 

Q: Does this require a C&T prior to initial approval with HCPF?
      A: Yes, after accepting the initial Adult Day Services application, CDPHE will conduct an initial survey. Following a zero deficiency survey, a C&T will be created and uploaded to COHFI. The facility/agency will then need to provide that C&T to HCPF as an attachment to their Medicaid Provider Enrollment Application.
 

Additional Information: This is a non-residential service that requires a facility location. Adult Day Services cannot operate within a licensed health facility/agency unless the Adult Day has separate and distinct space, entrance, and does not require consumers to travel through licensed space to reach the Adult Day location.

    A: Yes, after accepting the initial Alternative Care Facility (ACF) application, CDPHE will conduct an initial survey. Following a zero deficiency survey, a C&T will be created and uploaded to COHFI.          A: Yes, a Home Care Agency license is required. Either a non-medical class B or a medical class A Home Care Agency license is acceptable.

         A: Yes, existing licensed Home Care Agencies (HCAs) should submit a Letter of Intent (LOI) to add In-Home Support Services (IHSS). Please Note: Applicants for IHSS must also be approved for Specialty Code 666 Personal Care/Homemaker CFC services. Providers applying for IHSS who are not currently approved for 666 Personal Care/Homemaker must also apply for 666 Personal Care/Homemaker services.

Q: Is a CDPHE HFEMSD license required?
         A: Yes, an Assisted Living Residence (ALR) license is required. Providers must also be contracted with the Colorado Department of Human Services (CDHS) to provide Level 1 care.

Q: Do I need to submit an application to CDPHE?
         A: CDHS is not currently entering into any new contracts with Level 1 providers at this time. This program will eventually be transitioned to the Behavioral Health Administration (BHA).

Q: Does this require a C&T prior to initial approval with HCPF?
         A: Yes, if a provider is approved through CDHS and obtains an Assisted Living Residence license, CDPHE will conduct an initial survey. Following a zero deficiency survey, a C&T will be created and uploaded to COHFI. The facility/agency will then need to provide that C&T to HCPF as an attachment to their Medicaid Provider Enrollment Application. Providers cannot be certified for both 602 Alternative Care Facility and 755 Mental Health Transitional Living Homes.
