Licensure Fee Information

• Full facility/agency fee schedules can be found on our Facility Fees Page which will be updated annually by March 1st to reflect new fees for the next fiscal year.
• From 3/1 to 6/30 each year, there will be two fee schedules active. During this time the 3/1 - 6/30 Fee Transition Period Guidance document will help guide you on the appropriate fees to pay for your specific circumstances.

Home and Community Based (HCBS) Medicaid Waiver Services

The Home and Community Based (HCBS) Waiver Services - Medicaid Certification page will provide information on various Home and Community Based (HCBS) Services for which CDPHE is involved.

• Medicaid Certification - Home and Community Based Services & Application Processes
• The CDPHE Health Facilities and Emergency Medical Services Division does not charge fees for Medicaid Certification applications, however Health Care Policy and Financing (HCPF) may charge a fee for their Provider Enrollment process.
• Not all HCBS programs require an application to be submitted to CDPHE. Please review the requirements specific to the service you are wanting to apply for.
