A Request for Applications (RFA) is used to award grants to promote specific program goals. An organization (in this case, the Colorado Department of Public Health and Environment) announces that grant funding is available, and eligible organizations may apply for it. The purpose of this CDPHE RFA is to identify a Fiscal Agent for the West Regional Healthcare Coalition (HCC). The HCC is a loose confederation held together by joint goals for addressing preparedness and response goals within the health and medical field. The HCC comprises jurisdictional authorities in emergency management and local public health. This includes private organizations such as hospitals, emergency medical services, long term care facilities, and behavioral health centers, as well as schools, community health centers, and a host of other health centric community resources. The HCC is working toward building four key capabilities: 1. Foundation for health care and medical readiness; 2. Health care and medical response coordination; 3. Continuity of health care service delivery; and 4. Medical surge.
Eligibility:
Organizations that may apply for funding must meet the following criteria:
- Fiscal Agent must agree to perform all components of the Scope of Work(s) in partnership with the HCC Steering Committee and Member Organizations (as outlined in Attachment B).
- Fiscal Agent must be located within Colorado and preferably within one of the following 6 or neighboring Colorado counties: Montrose, Gunnison, Delta, Ouray, San Miguel, and Hinsdale.
- Fiscal Agent must be either affiliated with or be a member of the healthcare industry. A member of the healthcare industry is defined as an entity which provides medical services to clients.
- Fiscal Agent must have the ability to, upon award, become an active member within the West Region HCC. To be an active member, the awarded Fiscal Agent must be able to attend at least 75% of all regularly scheduled general meetings of the West Region Healthcare Coalition.
- Upon award, Fiscal Agent must have a demonstrated fiscal agent relationship with the West Region HCC.

The Fiscal Agent is responsible for:
- Hiring at least one (1) FTE covering the following two roles: the Readiness and Response Coordinator and Clinical Advisor.
- Creating in partnership with the HCC Steering Committee/Governance Board a sub-awardee agreement plan for implementation of all activities and deliverables.
- Managing the day-to-day fiscal activities of the HCC.
- Assuring all activities and deliverables are completed to standards.
- Provide to the HCC Steering Committee a monthly budget update.
- Facilitating budget workshops to plan for new budget periods.
- Reimbursing HCC member organizations for pre-approved purchases in a timely fashion.
- Submitting standardized invoices to CDPHE for reimbursement.
Note: This RFA does not support funding research, clinical care, furniture or equipment, construction, lobbying activities, or vehicle purchase.
This program has $266,927 available for this RFA, and CDPHE anticipates awarding up to one grant. Each contract will be effective upon approval by the State Controller through June 30, 2023. All aspects of the project are dependent upon funding availability.
