Facts for Action: Chronic Diseases and Related Risk Factors in Colorado

Dissemination of a series of fact sheets intended to increase access to and utilization of data to improve planning and evaluation of chronic disease-related programs in Colorado.

Interactive Indicator Inventory (a.k.a. “Triple-I”)

A web-based data dashboard being built to more widely disseminate chronic disease-related data, including trends in prevalence and disparities by age, sex, race/ethnicity, poverty level, education level, and county/region of residence.

Healthy Eating Active Living (HEAL) Surveillance Work Group

A working group with members representing state and local public health agencies and organizations with an interest in monitoring HEAL data, with the objectives of creating and collaborating on a surveillance plan and a plan for dissemination of data and information.

Evaluation of the chronic disease surveillance system

An evaluation of the current data being monitored in terms of simplicity, flexibility, representativeness, timeliness, and stability and whether data dissemination meets stakeholders’ needs.
