We are currently processing death certificate changes or corrections received on or before:

Beginning January 1, 2026, there will be a change in the fees for our services. See application fees for more information.
By mail
To request a correction to a death certificate, you must submit the following:
- Completed Death Certificate Correction form.
- Copy of your identification and eligibility.
- Documentary evidence of the change or correction.
- Non-refundable processing fees.
In-person
Our office is open to the public by appointment only. You must schedule an appointment in advance, no walk-in services will be available.
