Thank you for your interest in becoming a CDPHE-Recognized health navigator training program. Once “Interim” approval is granted, the name, contact information and pass rate for the training program will be listed on the Colorado Health Navigator Registry.

  1. Download and complete the application for CDPHE-Recognition: Application form.

  2. Get a written curriculum authorization release (if applicable): If your curriculum was developed by a third party, request an email or letter from the developer granting permission to use the curriculum to train health navigators.

Getting health navigators listed on the registry

It is the responsibility of the CDPHE-Recognized training program to have each health navigator who would like to be listed on the Colorado Health Navigator Registry sign and submit an Attestation of Health Navigator Training and Competency Assessment form. This form will be provided to training programs once they are granted “Interim” status as a CDPHE-Recognized training program.

Application Review Committee

How decisions will be made regarding meeting application criteria

As per the Colorado Standards and Requirements for CDPHE Voluntary Recognition of Health Navigator Training Programs, staff will first review the application and grant interim approval. The staff will then meet with the application review committee as a group to determine if they concur with the interim approval and whether to grant full approval. Decisions will be made based on the completeness of the application and whether the standards are met. A review checklist will be used to assess the application. If any information is missing or unclear to the staff or committee members, the training program will be contacted to provide additional information.

The application review is not a scoring process. In other words, the review is not the same as a grant review where individuals read a proposal and then score it and do not have an opportunity to gather more information and communicate with the applicant. In fact, the process does not include any scoring or rating. The process is more like that used for other health professions’ training both at DORA and CDPHE. The applicant either meets the standards and requirements or does not. If it does not, there is an opportunity to provide additional information. The staff will work with the programs to assist them in becoming approved, including conducting a site visit.
